自殺企図による頸部切創の4症例

Four cases of neck injury due to suicide attempt

Ⅰ．はじめに

　頸部外傷は救急の現場において遭遇する可能性のある疾患の1つである。われわれ耳鼻咽喉科医は頸部の構造を熟知しその対応については日頃より準備をしておく必要がある。頸部は気道，血管，神経，頸椎，食道など多くの重要臓器を有しており，創の状態によっては喉頭気管損傷による気道閉塞，血管損傷による出血性ショックなどを生じやすく致命的となる可能性がある。さらに救命救急センターを有する施設では頸部外傷のみならず他部位の損傷も合併することが多く，見逃さないよう注意を払う必要がある。また時には全身管理の必要もあるため局所所見にとどまらず全身状態を的確に把握することも必要である。われわれは2004 ～ 2006年までに自殺企図による頸部切創を4例経験した。ショック状態の症例や頸部以外の損傷も3例に認めたので若干の文献的考察を加えて報告する。

　We report four cases of neck injury due to suicide attempt from 2004 to 2006. The patients were 3 males and 1 female,with ages ranging from 24 to 86 years. One patient had neck injury alone,while the other 3 patients had additional injuries including the wrist,abdomen and chest. They used cutter,paring knife,kitchen knife or razor. The external jugular vein was injured in 2 patients,and the hyoid bone was exposed in 2. One patient cut into the pharynx. Two patients had depressive disorder and one had dementia. They were all properly treated and presently have no functional disorder in the neck.
