頰部放線菌症の1例

A case of actinomycosis in the cheek

I．はじめに

　放線菌症は口腔内や消化管に常在する放線菌による化膿性感染症であり，典型例では板状硬結，多発性小膿瘍，開口障害など特徴的な臨床症状を呈する疾患である。1990年代前半までは本症の多数例に対する報告が数多くみられるが^1～3），その後は1例報告が散見される程度である。1990年代に抗菌薬の使用頻度が増し典型例が減少していること，放線菌症の治療経験をもつ医師が減少し非特異的炎症として扱われていることなどが原因として考えられる。今回われわれは，頰部に発症した放線菌症の1例を経験したので文献的考察を加えて報告する。

　A 71-year-old woman complained of a swelling in the right cheek without pain. Pathological examination revealed actinomycotic bodies in granulation tissue,although actinomyces was not detected with bacterial investigations. The patient was treated with oral AMPC for 3months,and no recurrence of inflammation was found. Additionally,we investigated 24 patients of maxillofacial actinomycosis reported in Japan from 1995 to 2004.

　Recently,due to abuse of antibiotic drugs,diagnosis of actinomycosis has become difficult and the number of patients with typical symptoms has decreased. It was suggested the pathological examination for the definitive diagnosis was required.
