有茎性腫瘤の喉頭内嵌頓によって呼吸困難をきたした下咽頭「いわゆる癌肉腫」症例

Carcinosarcoma in the Hypopharynx―A Case Report―

I．はじめに

　「いわゆる癌肉腫」（so-called carcinosarcoma）は癌組織が脱分化を起こし，一見肉腫様の間質を有するようになった腫瘍である。特徴として有茎性腫瘤を呈することが挙げられる。

　今回われわれは，「いわゆる癌肉腫」の症例で梨状陥凹深部に存在していた有茎性腫瘤が喉頭内に嵌頓をきたし，突然の呼吸困難を呈した1例を経験したので，文献的考察を加えて報告する。

　Carcinosarcoma rarely occurs in the hypopharynx.

　A 75-year-old man,who complained of pain on swallowing,had laryngoscopic examination and was found to have a tumor localized in the right recessus piriformis,and the air way remained intact. Histopathological findings revealed squamous cell carcinoma. However,he was attacked by serious dyspnea after a series of chemotherapy. Reexamination showed a head of the tumor was stuck in the larynx,therefore he needed to have urgent tracheostomy. Total laryngectomy was finally performed and histopathological diagnosis was corrected to be carcinosarcoma.
