急性喉頭蓋炎の臨床的検討

Clinical Analysis of Acute Epiglottitis

I．はじめに

　急性喉頭蓋炎は声門上部に発症する急性感染症で，炎症は喉頭蓋から披裂部に及ぶ。ときとして急激に上気道閉塞をきたし，気管切開が必要となり，臨床上，注意が必要である。

　今回われわれは，横浜市立大学医学部附属病院と横浜市立大学医学部附属市民総合医療センター耳鼻咽喉科で加療をした急性喉頭蓋炎症例96例の背景因子，臨床症状などを検討した。本疾患の特性を再認識し，また，気管切開を施行するに当たって診断基準の一助となる因子をretrospectiveに検索し，その有用性を検討した。

　We reviewed 96 patients of acute epiglottitis in our hospital between January 1992 and December 2003. There were only three children under ten years old. No seasonal variation in the incidence of acute epiglottitis was encountered. 76 patients（79％）were admitted immediately or within 48 hours after commencement of symptoms. 91 patients（95％）received intravenous antibiotics and 80 patients（83％）were also treated with ste-roids. 8 patients had emergency tracheotomy and endotracheal intubation was not done.
