咽頭痛，嚥下困難にて耳鼻咽喉科を受診した破傷風の1例

A Case of Tetanus Presented with Sore Throat and Dysphagia

I．はじめに

　破傷風は，土壌に広く分布する破傷風菌（Clostridium tetani）の産生する毒素により，運動神経系の興奮，腱反射亢進から全身の痙攣発作をきたす疾患である。3種混合ワクチンやトキソイドなどの普及により，その発生数は減少しているものの，発症した場合には経過が急速で，適切な処置を行わないと死亡する場合もある。一般に本症は，外傷の既往とそれに引き続いて起こる開口障害，頸部硬直などの典型的な症状が出現すれば，診断は比較的容易である。しかし，その前駆症状である咽頭痛，嚥下障害などで耳鼻咽喉科を受診することがあり^1,2），初期症状を見逃さないためにも注意が必要である。

　今回われわれは，咽頭痛，嚥下困難で耳鼻咽喉科を受診し，その後の経過観察中に開口障害，頸部硬直が出現して破傷風と診断した症例を経験したので，若干の文献的考察を加えて報告する。

　A 70-year-old female presented with sore throat,dysphagia. The patient was diagnosed as having pharyngitis and admitted to our hospital. From the fourth day of admission convulsion occured,and continued for 14 days. The patient was treated with tetanus antitoxin,antibiotics,sedatives and respiratory management. Because tetanus is a serious disease,the possibility of this diagnosis should be kept in mind in patients presenting with severe sore throat and dysphagia.
