伐採中の竹による眼球破裂の1例

A case of eyeball rupture caused by a bamboo stick

　46歳女性が竹を伐採中に，しなった竹が左眼にあたり，即日受診した。強膜裂傷と硝子体脱出があった。強膜異物を除去し，鼻側の輪部から内直筋付着部後方に至る長さ9mmの裂傷を縫合し，強膜輪状締結を行った。残存する脈絡膜血腫と硝子体出血に対し，2週後に硝子体手術，脈絡膜血腫排出，水晶体切除，空気灌流，SF₆ガス注入を行った。左眼矯正視力は0.3に改善し，良好な結果を得た。植物による外傷としては異例の高度な眼球障害であったが，早期の適切な加療で，眼組織の障害を最小限に抑え，視機能の維持ができた。

A 46-year-old female was hit by a rebounding bamboo stick in her left eye. When seen the same day, the left eye showed scleral supture with vitreous prolapse. We removed the foreign body, sutured the ruptured sclera and performed scleral encircling. The residual vitreous hemorrhage and subchoroidal hematoma were treated by vitrectomy, aspiration of subchoroidal hematoma, lensectomy, fluid-air exchange and intravitreal SF₆ two weeks after the incident. The injured eye regained the final visual acuity of 0.3. This case illustrateds that severe eyeball trauma may result in fair outcome after immediate and adequate means.
