40歳の男性で,右眼眼底にgrouped pig—mentationを認めた1例を報告した。眼底の耳側半分に境界明瞭で,平坦な,黒褐色の色素斑を多数認め,色素斑は群生傾向を示し“bear tracks”あるいは“footsteps in the snow”と呼ばれる特有な所見が認められた。EOGとERG検査は共にsubnormalを示し,視野検査では色素斑に一致して感度の低下を認めた。このことから,網膜色素上皮細胞の機能異常と,これに続発した,病巣部の感覚網膜の障害を生じているものと推測した。
We observed grouped pigmentation of the retina in the right eye of a 40-year-old male. The pigment-ed spots were well-demarcated, appeared grayish black, were surrounded by hypopigmented halo and were distributed over the temporal hemisphere of the fundus. Those near the optic disc were smaller and those near the periphery tended to be larger. Large spots were often accompanied by smaller ones, manifesting a pattern simulating bear tracks.Fluorescein angiography showed blocked fluores-cence in areas of the spots and window defects or staining in the hypopigmented halo.Automated static perimetry showed decreased sensitivity in retinal areas corresponding to the pigmented spots. Electroretinogram and electrooculogram showed suppressed responses. The findings seemed to sug-gest degeneration of photoreceptor cells overlying the spots in addition to dysfunction of the hypertro-phied retinal pigment epithelium.
