当院に導入された汎用コンピューターを用いた総合医療情報システムの紹介

Description of computerized total medical information system in our hospital

　本学附属病院では1989年1月4日より，汎用コンピューターを用いた総合医療情報システムが導入された。まず人退院および給食オーダー・システムが稼働したのに続き，5月からは薬剤処方オーダーが病棟，外来の順でスタートした。今後は検査オーダー，放射線オーダー、病名オーダー，さらに将来はカルテの電子画像化なども予定されている。

　本システムの特徴は，病院内の金業務を包括し，各部所間を正確で質の高い情報によって相互に結ぶことにある。ところがシステムがフル稼働していない現在，われわれ医師にとっては手間のかかる人力作業のみが中心となり，そこから還元される情報はごくわずかなものにすぎない。さらに，眼科の診療形態を考えた場合，端末機の操作自体に他科以上の負担が強いられているようにも思う。

　本稿では，当院のシステムを紹介するとともに，眼科医の立場から現時点での問題点を挙げ，理想のトータルシステムとはどうあるべきかを考えてみた。

We introduced a total medical information sys-tem utilizing a general-purpose computer in our hospital 10 months ago. Initial functions included admission/discharge and provision of meals for in-patients. Ordering of prescription for both in-and outpatients started 5 months later. Future plans include ordering laboratory and radiological examinations, registration of diagnosis, and elec-tronic recording/retrieval of clinical charts.

This system aims at including all the hospital activities and at exchange of informations at an on -site basis. As the system in its current state han-dles a limited portion of the activities and informa-tions, not much benefit is apparent to the doctors involved. A major drawback appears to be that the ophthalmologists have to operate the computer terminals with resultant rather heavy work load.
