A 11-year-old boy presented with bilateral proptosis since 2 months before. He had had right fa-cial palsy 14 months before, which improved 2 months later. His visual acuity was normal in either eye. Fun-duscopy showed marked swelling of the optic disc in both eyes. Fluorescein angiography showed compatible findings. Computerized tomography showed bilateral swelling of the optic nerve. Critical flicker fusion fre-quency was slightly decreased in both eyes. His visual acuity decreased to 0.7 right and 0.4 left 8 days after his initial visit. He was diagnosed with sarcoidosis after biopsy of pigmented nevus in his hand showed charac-teristic granuloma. Peroral prednisolone was followed by improvement of optic disc swelling 2 weeks later. Facial palsy and proptosis improved another 2 weeks later. It appeared that optic disc swelling was induced by impaired axonal transport due to sarcoid granuloma.
