不顕性感染を認めた流行性角結膜炎の院内感染

An intrahospital outbreak of epidemic keratoconjunctivitis including subclinical infection

　2000年9日から3か月間に，当病院で流行性角結膜炎の院内感染が起こった。入院患者全員と本症感染が疑われる外来患者に免疫クロマトグラフィ法を行い，ウイルス陽性である患者50名を検索した。入院患者18名，外来患者32名である。50名中14名が自覚的に無症状であり，うち3名で細隙灯顕微鏡検査で流行性角結膜炎が否定された。ウイルス陽性であった50名から症状の有無にかかわらず5名を無作為に選択し，ウイルスの分離と同定を行い，全員からアデノウイルス8型が同定された。以上から，同一のウイルスによる感染でも症状に個体差があることと，角結膜炎所見がない患者への対処が二次感染の予防に重要であると結論される。

There was an intrahospital outbreak of epidemic keratoconjunctivitis (EKC) during 3 months through No-vember 2000. All the hospitalized patients and suspected outpatients underwent immunochromatography. Fifty pa-tients were positive for adenovirus infection, of which 14 were free of subjective symptoms. Slit-lamp examinations showed absence of EKC in 3 of the 14 patients. We performed virological examinations in randomly selected 5 out of the 50 patients. Adenovirus type 8 was isolated in all the 5 patients. The findings show that symptoms may be variable among individuals when infected by the same strain of adenovirus and that apparently symptom-free infected patients need particular attention in order to prevent further epidemics.
