We performed cataract surgery on the left eye of a 66-year-old female with insertion of foldable silicone intraocular lens. The immediate postoperative course was uneventful. Endophthalmitis with iritis as the major sign de-veloped 2 months later. Vitreous surgery was ineffective. Endophthalitis subsided following removal of intraocular lens and lens capsule 5 months after surgery. Culture of aqueous humor and vitreous was negative throughout. Electron mi-croscopy showed biofilm formation with cocci on the anterior surface and with rod-shaped bacteria on the posterior surface of the intraocular lens. The haptics showed biofilm formation with cocci and rods. The clinical course and ultra-structure findings were highly suggestive of bacterial biofilm infection as the cause of endophthalmitis.
