白内障術後眼内炎から摘出した眼内レンズの電子顕微鏡による観察

Electron microscopic findings of intraocular lens removed for endophthalmitis

　66歳女性の左眼に白内障手術を行い，シリコーン製のfoldable眼内レンズを挿入した。直後の経過は順調であったが，2か月目から虹彩炎を主体とする眼内炎が生じた。硝子体手術は無効であった。眼内レンズと後嚢混濁の関与を疑い，5か月目に眼内レンズと水晶体嚢を摘出した。これにより虹彩炎は寛解した。前房水と硝子体の培養は陰性であった。電子顕微鏡による検索で，眼内レンズの表面側に球菌，裏側に桿菌のバイオフィルム形成があり，支持部に球菌と桿菌のバイオフィルム形成があった。臨床経過と検査所見から，本症例はバイオフィルム感染症であると判断した。

We performed cataract surgery on the left eye of a 66-year-old female with insertion of foldable silicone intraocular lens. The immediate postoperative course was uneventful. Endophthalmitis with iritis as the major sign de-veloped 2 months later. Vitreous surgery was ineffective. Endophthalitis subsided following removal of intraocular lens and lens capsule 5 months after surgery. Culture of aqueous humor and vitreous was negative throughout. Electron mi-croscopy showed biofilm formation with cocci on the anterior surface and with rod-shaped bacteria on the posterior surface of the intraocular lens. The haptics showed biofilm formation with cocci and rods. The clinical course and ultra-structure findings were highly suggestive of bacterial biofilm infection as the cause of endophthalmitis.
