We reviewed 35 cases of primary orbital tumor which underwent tumor removal or biopsy during the past 14 years. We paid particular attention to the value of anterior approach. The route of orbitotomy was anterior 16 cases, lateral 3 cases, transcranial 4 cases, and transmaxillary 1 case. Total removal of tumor was achieved in 81% of cases following anterior orbitotomy and in 79% of 24 surgically treated cases excepting cases of biopsy alone. Anterior orbitotomy resulted in blepharoptosis and recurrence in one case each. These findings show that total removal of tumor can be expected by anterior orbitotomy when 1) the tumor is palpable, 2) the tumor does not extend to the orbital apex, 3) the tumor is encapsulated and not infiltrative, and 4) there is no adhesion of the tumor with the surrounding tissue.
