白内障／IOL手術と術後眼—近未来の諸問題

　白内障／IOL手術は，安定した術後成績を示し，20世紀眼科学の成果の1つである。しかしなお，超音波乳化吸引法に一定の頻度でみられる術中合併症，後発白内障など術後合併症，術後視力の質などの問題とともに，大量の症例が手術されることから，手術教育の問題が残されている。ここでは，これらの問題に関連し，レーザー手術など超音波乳化吸引法に代わる新手技の前臨床評価，対緑内障点眼薬により惹起される嚢胞様黄斑浮腫の成因，選択的COX2阻害非ステロイド点眼の可能性，および新しい手術観察システムである高感度高品質3D-TVシステムの白内障／I0L手術教育に対する応用について説明する。

Cataract/IOL surgery shows consistently good postoperative results, and is one of the successesof 20th century ophthalmology. With phacoemulsification, however, there is a regular incidence ofintraoperative complications, postoperative complications such as aftercataracts, and problems such as thequality of postoperative vision. At the same time, since a huge number of patients undergo this procedure,surgical training issues remain. In relation to these problems, we herein discuss the preclinical evaluation ofnew techniques such as laser surgery to replace phacoemulsification, the possibility of selective COX-2inhibiting nonsteroidal eyedrops in pseudophakic eyes, the mechanism of cystoid macular edema caused byanti-glaucoma eyedrops, and the application of new surgical observation systems using high-definition, high-quality 3D-TV for cataract/IOL surgery education.
