アトピー性皮膚炎に伴った子午線方向の巨大な裂孔を呈した網膜剥離の1例

Retinal detachment with meridional giant tear in a case of atopic dermatitis

　子午線方向の巨大な裂孔を呈したアトピー性皮膚炎に伴う扁平な網膜剥離の稀少な15歳男性の症例を経験した。原因裂孔は3時方向鋸状縁から上耳側血管アーケードにかけて子午線方向に位置しており，同方向に一致して水晶体混濁とチン小帯断裂が認められた。裂孔は経毛様体硝子体切除術と冷凍凝固により閉鎖し，術後8か月経過した後も網膜は復位している。アトピー性皮膚炎に伴う網膜剥離の報告は数多くあるが，その発生機序は明らかではない。本症例における局所的な水晶体混濁とチン小帯断裂の存在は，アトピー性皮膚炎における網膜裂孔の発症原因として機械的損傷との関わりを強く示唆するのと思れる。

A 15-year-old boy presented with shallow retinal detachment in his left eye. A giant retinal tear was located along the temporal meridian reaching from the ora serrata to the vascular arcade. He had been suffering from atopic dermatitis. A localized opacity of the lens and zonular rupture were located in the same meridian. The retina was reattached up to postoperative 8 months after vitrectomy and cryopexy. The presence of localized lens opacity and zonular rupture strongly suggests that mechanical injuries have been involved in the pathogenesis of giant retinal tear in the same meridian.
