A 15-year-old boy presented with shallow retinal detachment in his left eye. A giant retinal tear was located along the temporal meridian reaching from the ora serrata to the vascular arcade. He had been suffering from atopic dermatitis. A localized opacity of the lens and zonular rupture were located in the same meridian. The retina was reattached up to postoperative 8 months after vitrectomy and cryopexy. The presence of localized lens opacity and zonular rupture strongly suggests that mechanical injuries have been involved in the pathogenesis of giant retinal tear in the same meridian.
