We observed 8 eyes in 4 patients who developed visual impairment following laser in situ keratomileusis (LASIK) in other institutions. Best spectacle-corrected visual acuities in the 8 eyes were : 0.02, 0.3, 0.4, 0.6, 0.7, 0.8, 0.9 and 1.2. Five eyes showed astigmatism of -3 diopters or more. Five eyes showed flap lines, in two of which the lines were irregularly tortuous. Two eyes showed vesicular opacities beneath the flap, apparently due to reaction against foreign bodies. Three eyes showed thinning in the central cornea and no flash lines, suggesting flap loss or injury during or after surgery. The findings show that postoperative visual impairment may be associated with creating the flap during LASIK.
