（R1-8PM−10）　1994年4月から1999年8月までの5年5か月間に，川崎病院眼科で白内障手術を行った成熟白内障105例109眼について検討した。比較的若年者の多い膨化白内障と，老年者を主体とする核硬化白内障では術中，術後の経過に大きな差異があることより，全体を膨化群と硬化群として分類し，術式，術中・術後合併症，術後視力について1,536眼の対照群と比較検討した。術中合併症の頻度は，膨化群2.8％，硬化群16.4％，対照群1.3％という結果であった。安全，安易に考えられがちな白内障手術だが，成熟白内障手術は術中・術後合併症頻度が高く，適切な手術時期や術式選択の必要があると考えられた。

We reviewed 109 eyes in 105 patients who underwent surgery for cataract during the past 65 months. The series comprised 36 eyes with intumescent cataract and 73 eyes with sclerotic cataract. Another series of 1,536 eyes with non-mature cataract served as control. Intraoperative complications developed in 2.8% of intumescent cataract, 16.4% of sclerotic cataract and 1.3% of non-mature cataract. The findings show that surgery for mature cataract is associated with a higher risk of intraoperative complications.
