間質性肺炎治療中に発症したアシクロビル耐性角膜ヘルペスの1例

Acyclovir-resistant herpes simplex keratitis in a case under treatment for interstitial pneumonitis

（P-1-19）　57歳の女性が左眼の異物感で受診した。6週間前から，間質性肺炎に対してプレドニゾロンとシクロスポリンによる免疫抑制療法を受けていた。左眼に典型的な樹枝状角膜病変があり，角膜ヘルペスと診断した。アシクロビル眼軟膏と全身投与を行ったが，病変はさらに悪化した。病巣から単純ヘルペスウイルスが検出された。角膜擦過を繰り返し，初診の1か月後からトリフルオロチミジン1日6回の点眼を開始した。以後，角膜病変は軽快，治癒した。本症は，不適切なアシクロビルの使用が耐性株に移行させたと推定された。

A 57-year old woman presented with foreign body sensation in her left eye. She had been treated with systemic prednisolone and cyclosporin for interstitial pneumonitis since 6 weeks before. Her left eye showed typical dendritic keratitis. Her corneal lesion progressed further after treatment with systemic and topical acyclovir for 4 weeks. Herpes simplex virus was isolated from the corneal lesion. The corneal lesion started to improve and to attain ultimate cure following repeated corneal abrasion and instillation of trifluorothymidine 6. i. d. It appeared that improper use of acyclovir during the early phase of the disease led to development of acyclovir-resistant strain.
