眼瞼皮膚弛緩症に伴った眼瞼下垂の1例

A case of eyelid ptosis associated blepharochalasis

　右眼の眼瞼下垂を主訴として，18歳女性が受診した。幼少時より頻回に繰り返す霰粒腫などの眼瞼の炎症の既往があった。特徴的な皮膚所見から，眼瞼皮膚弛緩症が疑われた。MRIで右側の上顎洞嚢胞が検出され，嚢胞による眼球への軽度の圧排があった。眼瞼挙筋の機能障害が軽度であったので，通常の方法による上眼瞼挙筋短縮術を行い，眼瞼下垂が軽快した。本症例の眼瞼下垂には，反復する上眼瞼の炎症と，上顎洞嚢胞による後方からの圧排が原因として関係していた。

A 18-year-old female presented with ptosis of the right eyelid. She had a history of recurrent blepharitis and chalazion since childhood. Characteristic dermal findings led to the tentative diagnosis of blepharochalasis. Magnetic resonance imaging (MRI) showed a cyst in the right maxillary sinus apex causing mild compression to the right eyeball. Since she had mild levator dysfunction only, conventional shortening of the levator palpebrae muscle was performed with success. Blepharoptosis in this case was caused by two types of inflammation : recurrent superior blepharitis and compression-induced inflammation by the maxillary cyst.
