広範囲の角膜潰瘍に対する強角膜移植術

Lamellar sclerocorneal transplantation for extensive corneal ulcers

（C2-3-5）　通常の全層角膜移植術が不可能な広範囲の角膜潰瘍3症例に対して，強角膜を一塊として縫着する強角膜移植術を施行した。症例はいずれも潰瘍発症から長期間を経過した，穿孔か角膜ブドウ腫を伴った感染性角膜潰瘍で，本手術により視力は手動弁以下から最良矯正視力0.4から0.8までに改善した。眼圧は2例で術後上昇し，薬物治療を必要とした。フォトケラトスコープではおおむね良好な角膜形状を呈しており，また術後の内皮細胞密度は2,300から2,800cells／mm²を保っていた。シクロスポリンの投与により3症例とも現在まで拒絶反応は認めない。強角膜移植術は広範囲の角膜潰瘍に対する，視力回復を目的とした前眼部再建術の選択肢の1つとなり得る。

We treated three eyes with extensive corneal ulcer by lamellar sclerocorneal transplantation. All the eyes had started as infectious corneal ulcer and showed, additionally, corneal staphyloma. The visual acuity improved from hand motion or less to 0.4 to 0.8 postoperatively. Two eyes developed elevated intraocular pressure needing further treatment. Photokeratoscope showed almost normal corneal configuration. The postoperative corneal endothelial cell count ranged from 2,300 to 2,800 cells/mm². All the cases received systemic cyclosporin A and no graft rejection developed so far. Sclerokeratoplasty can be an alternative procedure to preserve the eyeglobe and improve the vision for widespread corneal ulcer.
