緑内障治療によって生じた偽眼類天疱瘡の2例

Two cases of pseudopemphigoid following antiglaucoma medications

（P3-3-9）　抗緑内障薬に起因ずると考えられる偽眼類天疱瘡の2症例を報告した。症例1は結膜嚢短縮，瞼球癒着からさらに遷延性角膜上皮欠損，角膜血管侵入および結膜上皮の侵入へと病態が悪化し，ステロイド薬の点眼および内服シクロスポリン点眼，持続閉瞼などを行ったが眼所見は改善しなかった。症例2は結膜充血，瞼球癒着から角膜上皮欠損へと進行したが，持続閉瞼にて角膜所見は改善した。発症機序には点眼薬の細胞毒性，もしくは点眼薬の投与が引き金となった免疫反応が考えられた。偽眼類天疱瘡はいったん発症すると難治性のため，初期兆候を見逃さず早期に対処することが重要と考えられた。

We observed two cases who developed pseudopemphigoid after prolonged topical medication for glaucoma with timolol, dipivefrin among others. The condition failed to improve in one eye of the first case despite anti-inflammatory agents and prolonged lid closure. The second case improved after similar treatment. The observed pseudopemphigoid appeared to have been induced by the cytotoxicity of antiglaucomatous agents or immune-related. We advocate early detection and treatment of this condition.
