眼類天疱瘡の2症例

Two cases of ocular penphigoid

　病理学的および免疫学的に眼類天疱瘡と診断された2症例を報告した。

　2症例共，両眼に結膜の強い癒着・瘢痕萎縮による皺嚢形成，瞼球癒着を起し，角膜は全面にわたり肉芽組織により覆われていた。このため視力は極度に低下しており，代用涙液およびステロイドの点眼，ならびに角膜移植により視力の回復を得た。病理検査にて，結膜には上皮下に水疱が認められ，症例2では，結膜の基底膜に自己抗体の沈着が螢光抗体法により確認された。しかし，血清中には，両症例共，皮膚粘膜に対する自己抗体は検出されなかった。

Ocular pemphigoid was diagnosed in two cases based on pathological and immunological bases. The cases, a 67-year-old male and a 50-year-old female, manifested severe shrinkage of the bulbar conjunctiva with the cornea covered by thick gran-ulomatous membrane bilaterally. Biopsy of the conjunctiva showed the presence of numerous small vesicles under the epithelium without acanthoysis in both cases. Auto-antibodies were demonstrated in the basement membrane of the conjunctiva in the second case. No antibodies were detected in the circulating blood in either case.
