Mass poisoning by sarin occurred in Matsumoto in central Japan on June 27, 1994. It involved over 500 victims including 7 deaths. Health check-up was conducted immediately, 13, 20, 33 and 45 months after the incident. Examinations included visual acuity, funduscopy, visual field, electroretinogram, pupillography and infrared optometry. Majority of cases showed normal pupillary size and light reaction except severely involved victims. Visual field constriction was present in one victim at 1 year and another severely affected victim showed miosis and subnormal electroretinogram at 45 months after the incident. Systemic manifestations included abnormal electroencephalogram and electrocardiogram, polyneuropathy and posttraumatic stress syndrome. Many cases showed persistent eye fatigue and impaired visual acuity.
