乳頭新生血管を生じたHLA-B27陽性前部ぶどう膜炎の1例

A case of HLA-B27-associated uveitis with optic disk neovascularization

（P3-1-13）　28歳男性が右眼の霧視で受診した。右眼にフィブリン析出，前房蓄膿，前房出血，虹彩後癒着を認めた。原因検索より，HLA-B27陽性ぶどう膜炎で強直性脊椎炎を合併していた。副腎皮質ステロイド薬（ステロイド）の内服および結膜下注射．点眼で炎症は軽快し，視力も改善した。しかしその後，右眼底に乳頭新生血管を生じていた。ステロイドの局所治療で新生血管は消退した。HLA-B27陽性ぶどう膜炎では後部病変にも十分な経過観察が必要である。

A 28-year-old male presented with blurring of vision in his right eye since two days before. The affected eye showed posterior synechiae, fibrinous exudate, hypopyon and hyphema in the anterior chamber. Further studies led to the diagnosis of HLA B-27-positive acute anterior uveitis and ankylosing spondylitis. The visual acuity improved following treatment with systemic and topical corticosteroid. Two months after onset, optic disk neovascularization was detected, which regressed after topical corticosteroid. This case illustrates that neovascularization in the posterior eye segment is a liability in HLA B-27-associated acute anterior uveitis.
