インターフェロンにより増悪した糖尿病網膜症の1例

Aggravation of diabetic retinopathy in a case under interferon therapy

　C型肝炎が発症したのちに糖尿病が発見された38歳男性が，当科を紹介された。視力は正常であり，前眼部，中間透光体，眼底に異常はなかった。インターフェロンαによる治療開始の2か月後，両眼底に軟性白斑と網膜出血が生じた。網膜症がさらに進行したので，汎網膜光凝固を行った。初診から9か月後，左眼に硝子体出血が発症し，硝子体手術で治癒した。本例は，糖尿病網膜症がインターフェロン投与で悪化したものと解釈された。

A 38-year-old male was referred to us for diabetes mellitus detected after diagnosis of C-type hepatitis. He had full visual acuity in both eyes. Ophthalmological examinations showed normal anterior segment, media and the fundus. Two months after initiation of treatment with systemic interferon, both eyes developed soft exudates and retinal hemorrhages. Panretinal photocoagulation was performed after the retinopathe further progressed. Massive vitreous hemorrhage developed 9 months later, which was successfully treated by vitreous surgery. This case illustrates that diabetic retinopathy may aggravate after systemic interferon treatment.
