植皮による義眼床作成後の術後管理の一工夫

Postsurgical care of reconstructed eyesocketive for prosthesis using skin graft

　植皮による義眼床作成後，移植皮膚片の拘縮予防のため，熱可塑性プラスチックを利用した装具を使用し，良好な結果を得た。本装具は作成が容易で微調整も可能なため，装具の脱出や皮膚潰瘍などの合併症なしに，再建した眼窩の形態に良好に適合する。本装具の使用により，瘢痕拘縮による眼窩容積の狭小化，上眼瞼陥凹や上・下眼瞼の内反などの義眼床作成後の合併症の予防に有効と思われた。また手術後管理の重要性につき再認識した。

After reconstructing an eye socket with skin grafting, it is important to apply pressure andmodelling maneuver to prevent contracture. We devised a splint for the eyesocket made of theremoplasticmaterials. It was applied to 5 eyes including 3 after injury and 2 after orbital tumor. This splint is light-weight, readily fits the reconstructed eyesocket, and resists the compression forces. After the scar hasstabilized a satisfactory volume of the after applying the splint, eyesocket is maintained and the depressionof the upper eyelid is improved.
