A 12-year-old girl was hit by the dull tip of thrown umbrella on her left upper eyelid. The visual acuity was hand motion when first seen 2 days later. The injured eye showed moderate hyphema, iritis and vitreous opacity which did not interfere with funduscopy. Commotio retinae was present in the inferior retina. The optic nerve head was reddish and edematous. It was hollow in its lower half portion. Fluorescein angiography showed delayed perfusion in the lower portion of the disc with disturbed choroidal circulation in the inferior hemisphere. Perimetry showed complete defect in the superior visual field. After two years of conservative treatment, the visual acuity improved to 0.2. This case of incomplete traumatic avulsion of the optic nerve was unusual due to minimum vitreous hemorrhage allowing detailed fundus observation.
