角膜縫縮術が著効した円錐角膜の急性水腫の1例

Corneal suture was effective in a case of acute hydrops secondary to keratoconus

　円錐角膜において急性水腫のために急激な視力低下をきたすことはしばしば経験するが，角膜穿孔に至ることは非常に稀である。従来，急性水腫の治療は保存的に経過観察することが多く，積極的な治療はあまり行われていない。筆者らは，円錐角膜の経過中に片眼に急性水腫より角膜穿孔をきたし，他眼に急性水腫を生じた症例を経験し，デスメ膜の修復を促進する目的で両眼に角膜縫縮術を施行した。その結果，実質浮腫を早期に消退させ，瘢痕形成を最小限にとどめることができ，良好な経過を得た。本術式は，急性水腫に対し積極的に行ってよい治療法であると考えられた。

We treated a case of bilateral keratoconus with acute hydrops in both eyes. Corneal perforation developed in the right eye. We placed mattress sutures with 10-0 nylon to bring the ruptured Desce-ment's mambrane into apposition. Postoperativehealing process was more prompt and favorable than conservative therapy, resulting in good visual acuity. We advocate this mode of therapy for acute hydrops in keratoconus because of its efficacy and minimum scar formation.
