糸状角膜炎に対する角膜表層穿刺

Anterior stromal puncture for filamentary keratitis

　従来の治療に抵抗した再発性の糸状角膜炎6例に対して角膜表層穿刺（anterior stromal　puncture）を施行し，全例で有効であった。症例は34〜80歳の男2例，女4例で乾性角結膜炎2例，角膜パンヌス2例，再発性角膜びらん1例，原因不明1例であった。Anterior stromal punc—　tureはdisposableの22ゲージ注射針を用いて行い，フィラメントの根部に2〜3回の穿刺を加えた後にフィラメントを除去し，再発するものには1週ごとに数回繰り返して穿刺を行い，圧迫眼帯を併用した。全例でフィラメントの著明な減少ないし消失がみられ，自覚症状は著しく改善した。本法は注意深く行えば安全かつ簡便であり，糸状角膜炎に対して有効な治療法であると考えられた。

We performed anterior stromal puncture of the cornea in 6 eyes of recurrent filamentary keratitis after conventional therapeutic modalities proved futile. The presest series included keratoconjun-ctivitis sicca 2 eyes, corneal pannus 2 eyes, recur-rent corneal erosion 1 eye and idiopathic filamentary keratitis 1 eye. The ages ranged from 24 to 80 years. In anterior stromal puncture, the root of the filament was punctured 2 or 3 times with a double-barbed 22-guage needle followed by the removal of filament. In recurrent cases, this proce-dure was repeated over weeks. Pressure bandage was occasionally applied. Anterior stromal punc-ture was effective in minimizing the filaments and relieving discomfort in all the treated eyes.
