網膜色素上皮掻爬を行った黄斑円孔術後のSLOスコトメトリー

Scanning laser ophthalmoscope scotometry after surgery for macular holes by scraping the retinal pigment epithelium

（展示201）　硝子体手術により黄斑円孔が閉鎖した53眼を対象として走査レーザー検眼鏡によるスコトメトリーを施行した。黄斑円孔底の掻爬を行った22眼では，掻爬範囲がスコトメトリーの視標サイズより小さな2眼を除いた20眼すべてで絶対暗点が検出された。一方，黄斑円孔底の掻爬を行わなかった31眼では，すべて絶対暗点が検出されなかった。網膜色素上皮掻爬による黄斑円孔閉鎖は単純に手術成功に含めるべきではないと考えられた。

We evaluated 53 eyes of idiopathic macular hole following successful vitreous surgery. Cure was achieved by vitreous surgery alone in 31 eyes and by additional scraping of the retinal pigment epithelium (RPE) in the hole area in 22 eyes. These eyes were studied by scotometry using a scanning laser ophthalmoscope (SLO). In the former group of 31 eyes, no absolute scotoma was detected in the hole area. In the latter group of 22 eyes, absolute central scotoma was detected in all except 2 eyes. The findings show that closure of macular hole after additional RPE scraping may not be regarded as surgical success.
