A 69-year-old female presented with neovascular glaucoma in her left eye. Central retinal vein occlu-sion had set in 4 weeks before. She had been under hemodialysis for the past 2 years. She had under-gone hysterectomy and cobalt radiation for uterus carcinoma 23 years before.

As a unique manifestation, the intraocular pres-sure in the affected eye showed an acute rise fromthe initial level of 30 mmHg to 50 mmHg and over during one session of hemodialysis. The plasma osmolarity decreased from the initial level of 308 mOsm to 285 mOsm at the end of hemodialysis.Concomitant intravenous use of hyperosmotic glycerol prevented fluctuations in plasma osmolar-ity and rise in intraocular pressure. A causal rela-tionship seemed to exist between intraocular pres-sure and plasma osmolarity in this patient.
