45歳男性の片眼白内障手術後に,Stevens-Johnson症候群が発症した。手術眼には発症前からベタメタゾンの点眼が行われていた。1年半の経過中,瞼球癒着の程度,パンヌスや混濁,瘢痕形成,潰瘍などの角膜病変,視力のすべてについて,手術眼の方が非手術眼よりも良好であり,ステロイド点眼が有効であったと評価された。本症では,メタゾラミド内服中に本症候群が発症し,これが誘因である可能性が推定された。
A 45-year-old male developed Stevens-Johnson syndrome following cataract surgery in his right eye. The right eye had received topical betameth-asone during the postsurgical period. During the clinical course of 18 months after development of Stevens-Johnson syndrome, the right eye fared consistently better regarding symblepharon, cor-neal lesions and the visual acuity. The observeddifference between the right and left eye seemed due to topical corticosteroid during the initial phase. The patient was under systemic treatment with methazolamide when Stevens-Johnson syn-drome became manifest. In view of similar occur-rence in 5 cases in Japan, use of methazolamide, a carbonic anhydrase inhibitor, would need due pre-caution in this context.
