肢端紅痛症に低体温を伴う白内障の手術例

Cataract surgery in a case with erythermalgia and hypothermia

　肢端紅痛症は，皮膚温の上昇に伴う四肢末端部の発作性の激痛と潮紅を伴うまれな疾患である。今回，この肢端紅痛症に低体温を伴った症例に，白内障手術を施行した。

　症例は18歳の男性で，両眼の霧視を主訴に当科を受診。患者は8歳の頃に下肢の熱感を認め，肢端紅痛症にて治療を続けており，9歳の頃にはステロイド投与を受けている。10歳の時，突然意識低下と低体温のために精査を受けたが原因は不明であった。初診時，視力右0.45（n．c．），左1.0（n．c．），両眼水晶体後嚢下に混濁を認めた。その後，徐々に霧視が増強し，視力も低下したため，右眼に対し白内障手術を施行した。白内障の原因は不明であるが，ステロイド白内障が最も疑われた。

A 18-year-old male had been suffering from hypothermia and erythermalgia in the lower extremities since 8 years of age. Systemic cortico-steroid therapy was started one year later. Visual acuity was impaired due to posterior cortical cata-ract. Cataract surgery in the more advanced eye was performed. The cataract seemed to be secon-dary to systemic corticosteroid therapy.
