老人性円板状黄斑変性症のクリプトンレーザー光凝固による治療成績

Krypton laser photocoagulation for senile disciform macular degeneration

　当科にてクリプトンレーザー光凝固にて治療を行った老人性円板状黄斑変性症患者43例44眼の凝固後の経過を観察した．治療成績は病型によって異なり，漿液性網膜剥離型が最も良く，網膜下結合織形成型，網膜下嚢腫型は光凝固を行っても悪化する症例が多かった．クリプトンレーザー光凝固は，特に中心窩に近接する部の網膜下新生血管網を凝固閉塞するのにアルゴンレーザー光凝固よりも有効であった．しかし，進行例では光凝固を行っても悪化する例が多いので，本症に対する治療は漿液性網膜剥離を主病像とし，まだ結合織増殖の少ない初期のうちに診断し，螢光眼底造影にて証明された新生血管網を十分凝固することが必要と思われた．

We treated a consecutive series of 44 eyes with senile disciform macular degeneration with the use of krypton red laser photocoagulation during the past 2 years. The effect of the laser therapy was excellent in the early stage of the disease in which serous retinal detachment was present before onset of the disciform response. In advanced cases, the visual acuity tended to deteriorate in spite of laser treatment. Red krypton laser proved to be adequate to conagulate the subretinal neovascular networks near the fovea provided the treatment be carried out in the early stage of the disease.
