肝移植後にサイトメガロウイルス網膜炎を併発した1症例

Cytomegalovirus retinitis developed in a case after liver transplantation

　成人のサイトメガロウイルス（CMV）網膜炎は，悪性腫瘍罹患中，移植後，副腎皮質ステロイド薬大量使用下，後天性免疫不全症候群など，免疫抑制状態にみられる。筆者らは，日本人で初めて肝移植を受けた50歳男性に発症したCMV網膜炎と思われる症例を経験した。両眼の視力障害にて来院。両眼に硝子体中の細胞とともに，後極部を中心に網膜血管に沿った黄白色滲出物と網膜出血等，壊死性網膜炎の所見がみられた。既往歴として肝細胞癌のため8か月前に米国にて肝移植を受け，経過中渡米した際，尿よりのウイルス分離とともに臨床的にCMV網膜炎の診断のもと，ganciclovir®を投与されている。本邦でも各種臓器移植の増加とともに日常診療での遭遇も多くなると思われる。

A 50-year-old male underwent liver transplanta-tion for hepatic carcinoma. Eight months later, he developed necrotizing retinitis in both eyes. Fundus-copy showed white yellowish exudate and retinal hemorrhage along the major retinal vessels in the central fundus. Cytomegalovirus was later isolated in the urine. The case was diagnosed as cytomegalovirus retinitis, and treated with sys-temic ganciclovir. Use of immunosuppressive agents and corticosteroid in massive dosis seemed to have contributed to the cytomegalovirus retinitis.
