Abstract Purpose:We report a case of hypotony maculopathy due to traumatic ciliary body dialysis, successfully treated by vitrectomy with pars plana cryopexy and gas tamponade.

Case:A 58-year-old woman presented with traumatic cataract of the left eye following blunt ocular trauma. She had previously undergone cataract surgery and was diagnosed with hypotony maculopathy due to a left intraocular pressure(IOP)of 4-6 mmHg, observed macular folds, and ultrasound biomicroscopy(UBM)revealing a void in the supra-choroidal space around the entire circumference. Eighteen months had passed since the trauma. After vitrectomy, combined with pars plana cryopexy and gas tamponade, her IOP stabilized and hypotony maculopathy resolved.

Conclusion:Vitrectomy with pars plana cryopexy and gas tamponade is a good treatment option for traumatic hypotony maculopathy. Even in cases such as this one, in where more than 1 year had passed since the eye trauma, normalization of IOP and preservation of visual acuity were considered possible.
