Abstract Purpose:This study presents a case of pediatric scleritis with myeloperoxidase antineutrophil cytoplasmic antibody(MPO-ANCA)positivity, accompanied by subretinal ridges. The utility of ultra-widefield optical coherence tomography(UW-OCT)for effective follow-up is highlighted.

Case:A 12-year-old girl presented with eyelid swelling, ophthalmalgia, and conjunctival congestion. Initial treatment with betamethasone and levofloxacin yielded no improvement, and no abnormal intracranial findings were observed. Owing to the suspicion of scleritis associated with systemic disease, the patient was referred to Shinshu University Hospital. Initial examination revealed visual acuity value of 0.9 and 1.2, with intraocular pressures of 13 mmHg and 15 mmHg, in the right and left eyes, respectively. Apart from scleritis, no anterior chamber inflammation, vitreous opacity, retinal exudate, or hemorrhage was observed. UW-OCT revealed the presence of subretinal ridges. Blood tests confirmed MPO-ANCA positivity, leading to a diagnosis of MPO-ANCA-positive scleritis. Oral prednisolone was initiated at 1 mg/kg, resulting in the improvement of eyelid swelling, ophthalmalgia, and UW-OCT findings. Oral steroids were tapered over 1 year, and no recurrence of symptoms was observed during the subsequent 2 years.

Conclusion:We encountered a case of pediatric scleritis with MPO-ANCA positivity. While there are limited case reports of MPO-ANCA-positive pediatric scleritis, it should be actively considered in the presence of intractable scleritis. UW-OCT, a non-invasive and easily performed test, is useful for evaluating the course of treatment for this condition.
