Abstract Purpose:To report a case of an intraocular foreign body(metal fragment)that penetrated deep into the choroid with only minor impact.

Case:A 21-year-old man complained of slight ocular pain in the right eye after a minor impact with an object in a warehouse.

Findings and Clinical Course:On initial examination, the visual acuity of the right eye was 0.9, and the intraocular pressure in the right and left eye was 16 mmHg and 17 mmHg respectively. A shallow anterior chamber and conjunctival edema were evident in the right eye, with a tear in the cornea at 7 o'clock. The fundus showed subretinal white lesions and preretinal and subretinal hemorrhage. Although no obvious perforation or intraocular foreign body was observed, an intraocular foreign body was suspected based on computed tomography. Subsequently, a choroidal tear extending to a white lesion near the macula was confirmed through vitrectomy. Examination of the white lesion revealed no intraocular foreign body, and the procedure was completed with silicone oil replacement. Postoperative computed tomography showed persistent intraocular foreign body in the eye, and the patient underwent reoperation. The white lesion was thoroughly examined deeper into the choroid, and the foreign body was located and removed with forceps. The retrieved foreign body was a cylindrical metal fragment, 3 mm in length.

Conclusion:Even a minor impact with an object can cause a foreign body to penetrate deep into the choroid, and come cases may require a deeper search of the choroid for a foreign body.
