Abstract Objective:We report a case of cystoid macular edema(CME) complicated by 0.002% omidenepag isopropyl ophthalmic solution after lens-sparing trabeculectomy.

Case:A 65-year-old female with bilateral normal-tension glaucoma underwent lens-sparing trabeculectomy. Six months after surgery, treatment with 0.002% omidenepag isopropyl eye drops and β-blocker eye drops was initiated. During a follow-up visit 1.5 months after starting these medications, the corrected visual acuity of the left eye decreased from 1.2 to 0.6. Optical coherence tomography(OCT) revealed CME in both eyes. Although the patient had a phakic eye, she was diagnosed with CME as an adverse reaction to 0.002% omidenepag isopropyl eye drops after iridectomy. Eye drops were discontinued and treatment with non-steroidal anti-inflammatory drug(NSAID) eye drops was initiated. One week after discontinuing the omidenepag isopropyl, the CME improved and after one month, it had completely resolved.

Conclusion:Patients who have undergone trabeculectomy with lens preservation and peripheral iridectomy are at risk of developing CME due to 0.002% omidenepag isopropyl eye drops, even in phakic eyes. After the onset of CME, discontinuation of eye drops and the use of NSAID may result in improvement.
