Abstract Purpose:To report a case of spontaneous suprachoroidal hemorrhage in a patient with macular hole retinal detachment.

Case:A 76-year-old woman with a history of bilateral high myopia developed vision loss in the left eye 3 weeks ago.

Findings:Her visual acuity was hand motion, intraocular pressure was low(3 mmHg), and the axial length was 29.17 mm in her left eye. Choroidal detachment extending circumferentially was noticed, but no retinal tear was observed. Ultrasonography revealed an internally hyperechoic choroidal detachment. We suspected retinal detachment with suprachoroidal hemorrhage in the myopic eye and performed a vitrectomy. A transconjunctival trocar cannula and vent tube were inserted, and a large amount of dark red blood was drained. We found a macular hole retinal detachment intraoperatively. Two months after the initial surgery, the macular hole closed, and the retina reattached.

Conclusion:Macular hole retinal detachment can be a cause of spontaneous suprachoroidal hemorrhage. The drainage of suprachoroidal hemorrhage using a trocar cannula and vent tube is a simple and useful treatment.
