Abstract. Purpose:To report a case of macular hole retinal detachment long after occurrence of traumatic macular hole. Case:A 31-year-old male presented with impaired vision in the right eye since 3 days before. He had been diagnosed with traumatic macular hole following blunt trauma to the right eye by flying baseball at the age of 17 years. He had received the same diagnosis 7 years before with visual acuity of 0.01. Findings:Corrected visual acuity was 0.01 right and 1.0 left. Both eyes were free from anomalous refraction. Intraocular pressure was 13 mmHg right and 15 mmHg left. The right eye showed macular hole with bullous retinal detachment in the inferior sector. The right eye was treated by vitrectomy, pneumatic retinal attachment, laser coagulation around the macular hole, and gas tamponade. The posterior vitreous was adherent to the retina as intraoperative finding. The retina became reattached with visual acuity of 0.02. Conclusion:Traumatic macular hole rarely causes retinal detachment after such a long interval of 50 years. Adherence of posterior vitreous to the retina was apparently a major causative factor in the present case.
