Abstract Purpose:The rho kinase(ROCK)inhibitor 0.4% ripasudil was added to cases of graft detachment after descemet stripping automated endothelial keratoplasty(DSAEK). Thereafter, we experienced two cases of good graft adhesion that did not require additional surgical treatment.

Case:Case 1 involved a loss of endothelium resulting from Fuchs endothelial dystrophy and exfoliation glaucoma in a 65-year-old woman. After glaucoma surgery performed by a local ophthalmologist, the patient developed bullous keratopathy(BK). A pupillary block occurred immediately after the DSAEK. Graft adhesion was poor due to air extraction. Therefore, air re-injection was planned. Two days after the administration of 0.4% ripasudil, the graft had adhered. Case 2 involved exfoliation glaucoma in a 75-year-old man. After multiple glaucoma surgeries at this hospital, DSAEK was performed due to the development of BK. Graft adhesion failure occurred after trabeculectomy due to low intraocular pressure. Air and 10% SF6 were injected, but the repositioning failed. A repeat transplantation was planned and a course of 0.4% ripasudil was started. One month later, the graft had adhered.

Conclusions:Here, we described two cases in which ROCK inhibitors effectively treated graft detachment after DSAEK. The mechanism of action may be the promotion of corneal epithelial migration, which could lead to recovered barrier function of the transplanted endothelium.
