視神経乳頭の発赤腫脹を主徴としたサイトメガロウイルス網膜炎の1例

Cytomegalovirus retinitis with optic disc redness and swelling in patients, with follicular lymphoma

要約　目的：濾胞性リンパ腫の患者に併発した視神経乳頭の発赤腫脹を主徴としたサイトメガロウイルス（CMV）網膜炎の1例を経験したので報告する。

症例：62歳，女性。左眼の視力低下を主訴に東京医科大学病院（当院）眼科に紹介となった。初診時の矯正視力は右眼1.2，左眼0.6で，左眼に視神経乳頭の発赤腫脹がみられた。

　前医で経過観察を行っていたが，MRIで左視神経にわずかな造影効果を認めたため，特発性視神経炎の診断のもと，ステロイドパルスを施行されたが視力の改善がなく，当院を再紹介され受診となった。当科再診時，視神経乳頭の発赤腫脹は改善していたが視神経萎縮と網膜滲出斑を認めたため，診断的治療目的で硝子体手術を行った。採取された硝子体液中のCMV-DNAが陽性となり，CMV網膜炎の診断に至った。

結論：悪性リンパ腫の治療後など，免疫能の低下した患者ではCMVによって視神経乳頭の発赤腫脹から始まる網膜炎を生じることがあり，鑑別疾患の1つとして留意する必要がある。

Abstract　Purpose：To report a case of cytomegalovirus retinitis with optic disc redness and swelling as initial ocular manifestations in patients with follicular lymphoma.

Case and Findings：A 62-year-old woman was referred to us for a complaint of visual loss in the left eye. Her corrected visual acuity was 1.2OD and 0.6OS. Her right eye was normal, but her left eye showed redness and swelling of the optic nerve.

　A pulse steroid therapy was administered under the diagnosis of idiopathic optic neuritis at the previous hospital, but no improvement in visual acuity was observed, and the patient was referred to our hospital again. At the time of re-examination, the redness and swelling of the optic disc improved, but optic atrophy with exudative retinitis was observed. Subsequently, a diagnostic vitrectomy was performed for diagnostic purpose. Cytomegalovirus DNA was detected in the vitreous humor, leading to a final diagnosis of cytomegalovirus retinitis.

Conclusions：In immunocompromised patients, such as those undergoing treatment for malignant lymphoma, cytomegalovirus may cause retinitis, which initially manifests as redness and swelling of the optic nerve, and should be considered as a differential diagnosis.
