Abstract Purpose:To report a case of cytomegalovirus retinitis with optic disc redness and swelling as initial ocular manifestations in patients with follicular lymphoma.

Case and Findings:A 62-year-old woman was referred to us for a complaint of visual loss in the left eye. Her corrected visual acuity was 1.2OD and 0.6OS. Her right eye was normal, but her left eye showed redness and swelling of the optic nerve.

 A pulse steroid therapy was administered under the diagnosis of idiopathic optic neuritis at the previous hospital, but no improvement in visual acuity was observed, and the patient was referred to our hospital again. At the time of re-examination, the redness and swelling of the optic disc improved, but optic atrophy with exudative retinitis was observed. Subsequently, a diagnostic vitrectomy was performed for diagnostic purpose. Cytomegalovirus DNA was detected in the vitreous humor, leading to a final diagnosis of cytomegalovirus retinitis.

Conclusions:In immunocompromised patients, such as those undergoing treatment for malignant lymphoma, cytomegalovirus may cause retinitis, which initially manifests as redness and swelling of the optic nerve, and should be considered as a differential diagnosis.
