Abstract Purpose:To report a case of bullous keratopathy with intractable round-shaped corneal ulcer that was diagnosed with herpetic keratitis by real-time polymerase chain reaction(PCR).

Case:An 86-year-old male was referred to us for bullous keratopathy in the left eye since 6 months before and corneal ulcer since 2 months before. Treatment as bacterial keratitis was reportedly futile. The left eye had received multiple surgeries for glaucoma.

Findings and Clinical Course:Round-shaped corneal ulcer with hypopyon was present in the left eye. Processing of corneal scrapings by PCR showed herpes simplex virus DNA and led to the diagnosis of herpetic keratitis. Topical treatment with acyclovir was followed by resolution of corneal ulcer in 2 weeks.

Conclusion:This case illustrates that herpetic keratitis may simulate bacterial keratitis and may persist treatment with antibiotics. Real-time PCR was useful in establishing the diagnosis.
