Abstract Background:Choroidal osteoma typically affects young females. It may develop atrophy of retinal pigment epithelium and decalcification of osteoma.

Purpose:To report choroidal osteoma in a 68-year-old female.

Case:A 68-year-old female was referred to us for abnormal fundus finding during regular health-check program. She had been receiving annual health check in the past and no problem had been detected about the eyes.

Findings:Corrected visual acuity was 1.2 in the right eye. An elevated lesion was present superior to the macula in the right eye. It was oval-shaped, orange-red in color, and was 4 disc diameters along the long axis. Fluorescein angiography showed hypofluorescence throughout. Optical coherence tomography showed decreased image. Computed tomography showed enhanced shadow along the posterior wall of the eyeball. These findings led to the diagnosis of choroidal osteoma in the right eye.

Conclusion:That the choroidal osteoma was not detected during the annual health check before the age of 68 years seemed to be due to the fact that the osteoma was in its early stage with calcification detectable only by CT and not by funduscopy.
