定額の選定療養費導入義務化に伴う眼科時間外受診状況の変化

Changes in the visit of outpatients after introduction of mandatory fee for registration

要約　目的：和歌山県立医科大学附属病院では2016年7月1日より定額（5,000円）の選定医療費徴収が開始され，選定療養費導入前後の眼科時間外受診状況の変化について検討を行った。

対象と方法：2016年7〜11月（平日102日，休日51日）の当院眼科の時間外受診患者（導入後群）（平日53人，休日93人）と，2015年7〜11月（平日102日，休日51日）の当院眼科の時間外受診患者（導入前群）（平日88人，休日112人）を対象として，1日平均受診患者数（平日，休日），12歳以下の割合について，それぞれ後ろ向きに統計学的に比較検討を行った。

結果：1日平均患者数は，選定療養費導入前後で平日はそれぞれ0.89±0.98人，0.54±0.78人，休日はそれぞれ2.08±1.47人，1.73±1.30人であり，平日の受診患者数は有意に減少（スチューデントのt検定，p＜0.01）したが，休日の受診患者数は有意差がなかった。休日の12歳以下の受診は導入前後でそれぞれ9人と16人であり，導入後で有意な割合増加（χ²検定，p＜0.05）がみられた。

結論：定額の選定療養費導入により患者の時間外受診数は減少したが，休日の12歳以下の受診割合は増加した。

Abstract　Purpose：To report changes in the time of initial visit of outpatients in our hospital after introduction of mandatory fee for registration as of July 2016.

Cases and Method：We counted the number of new outpatients during July to November 2015, and during July to November 2016. The former period comprised 102 weekdays and 51 holidays. The latter comprised 102 weekdays and 51 holidays each.

Results：Number of outpatients averaged 0.89±0.98 on weekdays and 2.08±1.47 on holidays before introduction of mandatory fee. It averaged 0.54±0.78 on weekdays and 1.73±1.30 on holidays after introduction. Thus, the number of outpatients decreased significantly on weekdays and not on holidays（p＜0.01）. Outpatients under the age of 12 years showed a significant increase on holidays after introduction of mandatory fee（p＜0.05）.

Conclusion：After introduction of mandatory fee, number of outpatients decreased significantly on weekdays and not on holidays. Outpatients under the age of 12 years showed a significant increase on holidays after introduction of mandatory fee.
