Abstract Purpose:To report a case of infiltrative optic neuropathy with diagnostic difficulties.

Case:A female in her late forties presented with impaired vision in her right eye since one week before.

Findings and Clinical Course:Best-corrected visual acuity was 0.02 in the right eye and 1.0 in the left. Findus findings were apparently normal. MRI showed mild accumulation of fluid in the right sphenoid sinus. After start of pulsed corticosteroid therapy, CT showed right sphenoiditis. Surgery led to the diagnosis of squamous cell carcinoma and to radiation therapy. Left visual acuity gradually decreased and became no light perception one year later. Systemic corticosteroid therapy was futile. Another 4 months later, the left eye developed oculomotor palsy. PET-CT showed recurrence of carcinoma extending from the primary lesion to the left optic nerve.

Conclusion:This case illustrates that infiltrative optic neuropathy may pose difficulties in diagnostic imaging. Systemic examinations, including cerebrospinal fluid studies, may be necessary in the diagnosis.
