Abstract Purpose:To present a report on the relation between anterior chamber depth and the lens in primary angle closure suspect(PACS).

Cases and Method:This study was made on 366 eyes of 201 cases who received routine examination including measurement of axial length. The age averaged 72.6±7.3 years. Intraocular pressure was within normal range in all the eyes. These eyes were classified into 3 groups after van Herick. Group 1 had axial length less than 2.0 mm(52 eyes). Group 2 had axial length equal to or more than 2.0 mm(74 eyes). Group 3 served as control(240 eyes).

Results:There was no difference among the 3 groups regarding age. All the parameters regarding the lens were significantly different between group 1 and group 2(p<0.01). These parameters were significantly correlated with anterior chamber depth.

Conclusion:Measurement of the axial length is useful in the diagnosis and treatment of primary angle closure disease. Eyes in group 1 showed high correlations with the lens factors. These eyes had also a high risk of angle closure.
