Abstract Purpose: To report a case of central nervous system(CNS)lymphoma followed by ocular lesions simulating paraneoplastic cloudy vitelliform submaculopathy.

Case: A 49-year-old male presented with impaired vision in the right eye. He had been diagnosed with CNS lymphoma lymphoma 30 months before that subsided after high-dose methotrexate treatment. The lymphoma seemed to have recurred recently.

Findings and Clinical Course: Corrected visual acuity was 0.1 in the right eye and 0.6 in the left. The right fundus showed a yellow granular lesion temporal to the macula. Optical coherence tomography(OCT)high reflection anterior to the retinal pigment epithelium. The fundus lesion disappeared after intravitreal injections of methotrexate. Similar fundus lesion developed in the left eye 3 months later with decrease of visual acuity to 0.02. Visual acuity improved to 0.9 in either eye after repeated intravitreal methotrexate.

Conclusion: This case illustrates that fundus lesion simulating paraneoplastic cloudy vitelliform submaculopathy may develop following CNS lymphoma and that it may respond to repeated intraviteal methotrexate.
