要約 目的:学校生活上の視機能評価に,Functional Vision Score(以下,FVS)での評価が有用であった先天無虹彩の1症例の報告。
症例:13歳,男性。先天無虹彩の患者が学校生活の問題を訴え受診した。
所見:矯正視力は右0.3,左0.4,眼圧は両眼18mmHg,先天性眼振,黄斑低形成であった。Goldmann視野検査は両眼とも正常。視覚の身体障害者手帳に該当しなかった。この症例に対し,FVSを用いて障害の程度を判定するとFVS=79.5で軽度視覚喪失であった。学校にFVSの結果を報告し,障害について理解を得ることができた。
結論:FVSで患者の不自由さを評価できる可能性が示された。
Abstract Purpose: To report a case of congenital aniridia who sought counseling as he had difficulty in school life due to poor vision. Evaluation by functional vision score(FVS)was useful for the patient as well as for the school and administration.
Case: A 13-year-old male complained of difficulty in school life due to impaired vision.
Finding: Best corrected visual acuity was 0.3 in the right eye and 0.4 in the left. Intraocular pressure was 18mmHg in either eye. He had nystagmus and macular dysplasia. Visual field was nearly normal by Goldmann perimetry. This case was not granted visually disabled by Japanese law but was classified to be mild vision loss by American Medical Association(AMA)method. We could report that he had mild vision loss to school and they understood his vision problem.
Conclusion: Even mild vision loss can be a big problem for a patient. FVS is a good tool to evaluate the degree of visual loss.
