28病日から免疫吸着療法を施行した抗アクアポリン4抗体陽性視神経炎の1例

A case of anti-aquaporin-4 autoantibody positive optic neuritis treated by plasma absorption starting four weeks after onset

要約　目的：第28病日から開始した免疫吸着療法が著効を示した抗アクアポリン（AQP）4抗体陽性の視神経炎の報告。症例：54歳の女性が3日前からの左眼の視力低下，視野欠損，眼痛で受診した。矯正視力は右1.2，左0.4であった。左眼には相対的求心路瞳孔反応障害が陽性であった。MRIで球後視神経炎視神経に相当する所見があり，第26病日に抗AQP4抗体が陽性であることが判明した。多発性硬化症や脊髄炎を示唆する所見はなく，視神経脊髄炎関連疾患である抗AQP4抗体陽性視神経炎と診断した。2クールのステロイドパルス療法は効果がなく，第28病日から免疫吸着療法を開始した。第46病日には左眼視力が1.0になり，以後314病日後の現在まで再発や脊髄炎はない。結論：抗AQP4抗体陽性視神経炎の症例に対し，ステロイドパルス療法は奏効せず，免疫吸着療法が著効を示した。

Abstract.　Purpose：To report a case of anti-aquaporin-4 autoantibody positive optic neuritis treated by plasma absorption starting four weeks after onset. Case：A 54-year-old woman presented with impaired vision, visual field defect, and ocular pain in the left eye since 3 days before. Best corrected visual acuity was 1.2 right and 0.4 left. The left eye showed relative afferent pupillary defect as the sole objective finding. Contract MRI showed findings suggestive of retrobulbar neuritis. Serological test showed positive for anti-aquaporin-4 autoantibody on day 26 of disease. Findings suggestive of multiple sclerosis or myelitis were absent. After two series of pulsed corticosteroid therapy proved futile, she started receiving plasma absorption on day 28 of disease. Left improved to 1.0 on day 46. There has been no recurrence until day 314 day of disease. Conclusion：The present case of anti-aquaporin-4 autoantibody positive optic neuritis failed to respond to pulsed corticosteroid and showed rapid improvement after plasma absorption therapy.
