Abstract. Purpose:To report a case of anti-aquaporin-4 autoantibody positive optic neuritis treated by plasma absorption starting four weeks after onset. Case:A 54-year-old woman presented with impaired vision, visual field defect, and ocular pain in the left eye since 3 days before. Best corrected visual acuity was 1.2 right and 0.4 left. The left eye showed relative afferent pupillary defect as the sole objective finding. Contract MRI showed findings suggestive of retrobulbar neuritis. Serological test showed positive for anti-aquaporin-4 autoantibody on day 26 of disease. Findings suggestive of multiple sclerosis or myelitis were absent. After two series of pulsed corticosteroid therapy proved futile, she started receiving plasma absorption on day 28 of disease. Left improved to 1.0 on day 46. There has been no recurrence until day 314 day of disease. Conclusion:The present case of anti-aquaporin-4 autoantibody positive optic neuritis failed to respond to pulsed corticosteroid and showed rapid improvement after plasma absorption therapy.
